Mr. Preston is a 63-year-old male that presents to the Emergency Department with pain in his bilateral lower extremities with walking and a non-healing ulcer on the big toe of the left foot. His past medical history includes a myocardial infarction, hypertension and Type II diabetes. He works as a handyman and does not have insurance. Lately, he has not been working due to the pain in his legs and his work boots causing the ulcer on his big toe to hurt. He buys his medications and sees a provider when he can afford it. He reports that this pain and limited budget have affected his diet as he often eats peanut butter sandwiches and orders takeout meals. Mr. Preston is 5'8" and 225 pounds. He reports tobacco use of 1-2 packs per day x 45 years.
